Teen in Argentina survives 3 inch nail shot into heart chamber with nail gun
A carpenter in Argentina who was accidentally shot in the heart with a three inch nail has miraculously survived.
The unidentified 19-year-old was rushed to hospital in the capital of Buenos Aires last Monday after the bolt was fired by a colleague into his chest.
He had been working at a wooden pallet factory in the Cañuelas area when the accident occurred.
A shocking X-ray image clearly shows the deadly device piercing his heart chamber.
The teen underwent two hours of surgery at El Cruce Néstor Kirchner Hospital.
Doctors decided not to remove the nail immediately as it was “acting as a stopper and preventing a hemorrhage,” said the hospital’s executive director Arnaldo Medina.
Instead, Dr. Marcelo Nahín told La Nacion newspaper, “’In the operating room, we proceeded to open the breast bone very carefully because there was a risk of hitting the nail and enlarging the heart wound. With the nail visible, we performed a manual removal of the foreign body, which was embedded in the anterior right ventricular.”
Nahín said Wednesday, “The patient has recovered well and he is now off the ventilator.”
According to Medina only three people in the world had ever survived such an injury – in the U.S., Australia and Poland.
But this was the first time they had ever seen it occur in Argentina.
Nahín also warned about the dangers of nail guns saying, “They are powerful and easy to operate as they increase productivity, but they are also responsible for around 37,000 visits each year to emergency rooms in hospitals in the U.S.”
Argentine Dr. Nahin leads miracle cardiovascular surgery in nail shot case
Dr. Nahin, Argentine cardiovascular surgeon with the Hospital El Cruce, was in charge of Rodriguez’s case.
In recent weeks, Jonathan Rodriguez, 19-year-old carpenter from Buenos Aires, Argentina, found himself between life and death after a three-inch nail was shot into his heart chamber. Only four people in history have survived this type of accident.
Now, Rodriguez is back at home, recuperating from the miracle surgery that gave him a second shot (no pun intended) in life.
Dr. Marcelo Nahin, Argentine cardiovascular surgeon with the Hospital El Cruce, was in charge of Rodriguez’s case.
VOXXI: Dr. Nahin, Jonathan arrived at the El Cruce-Nestor Kirchner Hospital with a pneumatic gun nail embedded in his heart from an accident at work. How was this nail placed to allow him to arrive alive, in the first place?
Dr. Nahin: This is a very interesting question because the placement of the nail and the angle of incidence were determinants so the patient could survive for about eight hours from the time of the accident until the surgery.
The nail made a perpendicular entry to the sternum bone and the heart. The tip of the nail went through skin, subcutaneous fat, bone and the heart’s right ventricle. The head of the nail and its first 3 cm were fixed to the sternum bone, which the nail went through like paper, due to the power of these pneumatic guns. The head of the nail stopped it from crossing the heart side to side. The remaining 5 cm were housed within the right ventricle of the heart.
If the nail had injured the heart tangentially, it is likely that the patient would not have survived.
VOXXI: What is the increased risk with this type of injury?
Dr. Nahin: The risk was very high as it drilled the heart, an organ with four chambers filled with blood, which is permanently contracting and dilating to eject that blood, through the body. Upon receipt of a perforating wound in one of his heart cavities, the patient began to lose blood, the risk was him bleeding to death or a hypovolemic shock. He was also at risk of an acute cardiac blocking and death of cardiogenic shock.
VOXXI: What was your role in the case and how did your team face the surgery?
Dr. Nahin: I directed the surgical intervention. We had confidence in our previous experience—cardiac surgeries and transplants—that gave us assurance and peace of mind to deal with this type of injury. At that time, we couldn’t assure any results, because we had no dimension of all the damage.
There were several factors that contributed so that today, Jonathan can be alive today. Firstly, his referral to a high complexity center, as is the Hospital where we work, and the fact that nobody had tried to pull out the nail were determining factors.
Secondly, surely, the fact that he’s 19 and his physical strength contributed to the success of the surgery. And finally, the fact that the intervention was carried out by cardiovascular surgeons was actually a fundamental condition for optimal results.
VOXXI: What does it mean for you today, the achievement of having literally saved the life of this boy?
Dr. Nahin: The fact of having saved the life of this young patient, who was practically sentenced to death, meant a great moral satisfaction to me that filled my soul with joy.
I was pleasantly surprised by the global impact of this case, it allowed me to take real dimension of what we had achieved in the operating room. Personally, it means great pride that I share with all the members of the hospital.
One of the things that most struck me when I read the international press was when they dealt in plural form: “Argentine doctors saved” or “Argentine surgeons achieved…” I was one of those surgeons to which they referred, and without intending it, I had contributed to the world speaking of Argentina.
I spent all week moved and touched by the large amount of samples of affection received by relatives, acquaintances and people who I did not know in Argentina and all over the world, through my professional Facebook page.
To conclude, I would like to say that the Hospital de Alta Complejidad is a model hospital in Argentina. It was opened five years ago and thanks to this surgery, has achieved worldwide attention towards the Argentina medicine.

Los médicos califican de milagrosa una operación realizada a un carpintero en Argentina para sacarle un clavo del corazón. El hombre se accidentó en el trabajo con una pistola de clavos, y ahora, con sus 19 años de edad, le sobran razones para estar contento.
Después de una operación de dos horas en un hospital de Buenos Aires, los médicos lograron extraerle un clavo de 7cm ó 3 pulgadas que tenía incrustado en el corazón.
El cirujano cardiovascular que realizó la operación explicó que es un verdadero milagro que el joven conservara la vida. “La cabeza del clavo estaba a ras del hueso, y todo el clavo había pasado los dos centímetro de hueso, y los restantes cinco centímetros se habían metido dentro del corazón”, describió el doctor Marcelo Nahín.
El accidente
El joven se accidentó con una pistola neumática para clavar, mientras trabajaba en una maderera. Otro operario manejaba la herramienta cuando esta se disparó.
Los médicos dicen que una de las claves para el éxito de la operación fue que no se le retiró el clavo inmediatamente, ya que funcionaba como una especie de tapón evitando una hemorragia. “Si alguien le hubiese intentado sacar el clavo, el paciente fallecía, no llegaba a tiempo”, agregó Nahín.
Los accidentes con pistolas neumáticas son comunes, por eso carpinteros como Ricardo Hernández toman sus precauciones. “Estas pistolas de clavos son como un arma, o sea, si uno está a punto de utilizarla, nunca la tiene que tener mirando para el cuerpo, y aún más si sabemos que está la válvula del compresor abierta”, dijo Hernández.
Miles de casos cada año
Estas herramientas son responsables de 37 mil visitas anuales a salas de emergencia en los Estados Unidos.
“Yo creo que es hasta más peligrosa que un arma de fuego porque el arma de fuego, el que la está manejando, ya sabe que es letal, pero en este caso el que maneja la pistola neumática, no tiene conciencia de la gravedad”, dijo el doctor Nahín.
Esta fue la primera vez que se realiza una operación como esta con éxito en Argentina, un logro que sólo se había obtenido en otros tres casos anteriores: Estados Unidos, Polonia y Australia.

A 19-year old carpenter, who was working at a wooden pallet factory in the Buenos Aires suburb of Cañuelas, was rushed to a hospital in the capital after a coworker accidently shot a bolt from an electric nail gun three inches into his chest.
An X-ray of the carpenter’s chest shows the eight-inch bolt piercing his heart chamber.
What was almost certainly a deadly accident turned into a miracle after two hours of surgery.
Doctors did not immediately remove the nail as it was “acting as a stopper and preventing a hemorrhage,” said the hospital’s executive director Arnaldo Medina, according to the New York Daily News .
“In the operating room, we proceeded to open the breast bone very carefully because there was a risk of hitting the nail and enlarging the heart wound. With the nail visible, we performed a manual removal of the foreign body, which was embedded in the anterior right ventricular,” said the surgeon Dr. Marcelo Nahín .
Argentine doctors said that the carpenter is now recovering from the surgery and is off of a ventilator. Photos on the Argentine government Health Ministry’s website show the man, alongside Nahín, giving thumbs up.
There is tape covering the large incision on his chest.
“Only three people have survived this injury in the world, in the United States, Australia and Poland,” Medina said. “This is the first time this type of surgery has been performed in Argentina.”
Nail gun accidents are responsible for approximately 37,000 annual emergency room visits by construction workers in the United States. Most of these accidents, however, involve injuries to the fingers and hands, Nahín said.

During the more than two-hour operation, the challenge for the doctors was not to remove the nail, which acted as a stopper, and accidentally cause a hemorrhage.
Instead of pulling out the nail from the 19-year-old teen’s heart, doctors opened up the teen’s breast bone in order to remove the nail.
It was only through carefully opening up the breast bone that doctors could get a clear picture of the specific location of the three-inch nail in the teen’s heart. Sky News reports that according to lead surgeon Dr. Marcelo Nahin, the 19-year-old teen is recovering well.
While the teen who survived the shot to the heart is on the road to recovery, the teen’s story is a crucial reminder for everyone that foreign objects that accidentally penetrate someone’s body should not be removed because it could cause a bleeding out. Patients should always remain as still as possible to prevent a foreign object from doing any further damage.
Most First Aid classes teach that foreign objects like nails, knives, sticks, or rods should never be pulled from someone’s body but, at the most, only be secured.
The American Red Cross offers classes for several different categories like “Babysitting, Childcare, Bloodborne Pathogens, Emergency Preparedness, First Aid, CPR, AED for Lay Responders, HIV/AIDS Prevention Education, Lifeguarding, Small Craft Safety, Swimming Lessons, Water Safety Education, Family Care Giving,” and even Pet First Aid throughout the nation. Available classes can be found by zip code and category through the American Red Cross take-a-class search website.
Taking even just a short weekend First Aid class to learn how to handle penetrating foreign objects, like in the story of the teen who survived the shot into the heart, might make a difference in someone else’s emergency situation.

Bác sĩ Nahin cũng đưa ra những lời cảnh báo về nguy cơ của việc sử dụng súng bắn đinh. Theo ông, chúng rất mạnh và giúp nâng cao năng suất lao động cho con người nhưng lại rất nguy hiểm. Ở Mỹ, mỗi năm có khoảng 37.000 trường hợp phải cấp cứu tại bệnh viện vì thương tích do súng bắn đinh gây ra.

Die Ärzte konnten den Nagel nicht sofort entfernen, da er zunächst verhinderte, dass Blut aus der Wunde strömte, sagte Klink-Chef Arnaldo Medina zur Zeitung „New York Daily News“.
„Wir öffneten sehr vorsichtig den Brustkorb. Das Risiko war groß, den Nagel zu treffen und dadurch die Wunde am Herzen zu vergrößern“, erklärte der Chirurg Dr. Marcelo Nahin. Als sie den Nagel sahen, konnten sie den Fremdkörper entfernen und die Wunder schließen.
Der rund zweistündige Eingriff war erfolgreich: Der Zimmermann erhole sich nun von der Operation, sagen die Ärzte. Das argentinische Gesundheitsministerium veröffentlichte Fotos des Patienten und des Röntgenbildes. Auf der Brust ist eine lange Naht mit Klebeband abgedeckt.
Dass der junge Mann noch lebt, ist ein unglaublicher Glücksfall: „Nur drei Menschen haben diese Verletzung bisher überlebt. Einer in den USA, in Australien und in Polen“, sagte Klinik-Chef Medina. „Dies war das erste Mal überhaupt, das eine derartige Operation in Argentinien durchgeführt wurde.“

En 19-årig snedker, der ikke vil have sit navn frem, kom i mandags godt og grundigt galt af sted med en sømpistol i den argentinske hovedstad, Buenos Aires.
Den unge, argentinske snedker var tilsyneladende igang med at samle paller på en fabrik i Cañuelas-regionen, da han på noget uheldig vis fik skudt sig selv i hjertet med et cirka otte centimeter langt søm – noget som for langt, langt de fleste mennesker ender med at koste livet.
Det skriver flere internationale medier.
Men imod alle odds lykkedes det faktisk lægerne på El Cruce Néstor Kirchner Hospital at redde hans liv – blandt andet fordi man i første omgang lod sømmet sidde i hjertet.
– Vi startede med at åbne brystkassen meget forsigtigt og fjerne nogle knogler, så vi kunne komme til, forklarer lægen Marcelo Nahín til avisen La Nacion og fortsætter:
– Vi måtte være meget forsigtige, for hvis vi ramte sømmet kunne vi have gjort såret større. Først da sømmet var helt synligt, kunne vi manuelt fjerne det fra den højre ventrikel, siger han videre.
Selve operationen tog i alt to timer, hvorefter patienten blev lagt i respirator, hvor han hun er kraftigt i bedring.
Ifølge den lokale hospitalsdirektør Arnaldo Medina har kun tre andre mennesker i verden overlevet lignende operationer. Både i USA, Australien og Polen har man haft lignende patienter, oplyser han videre.

Foto X-ray menunjukkan paku 7,6 cm menembus lurus melalui otot jantung dan bersarang di salah satu biliknya. Ahli bedah di RS tersebut tidak serta merta mengeluarkan paku dari jantung pemuda itu untuk mencegah perdarahan.
Di ruang operasi, dokter ahli membuka tulang dadanya dengan hati-hati sehingga paku tersebut bisa terlihat dan diambil. Proses harus dilakukan dengan ekstra hati-hati agar tidak menimbulkan cedera pada jantung.
Dr Marcelo Nahin yang turut menangani operasi mengatakan pengambilan paku di jantung pemuda tersebut memerlukan waktu 2 jam. Setelah operasi dilakukan, kondisi pemuda itu membaik.

A carpenter in Argentina who was accidentally shot in the heart with a three-inch nail has miraculously survived.
The unidentified 19-year-old was rushed to hospital in the capital of Buenos Aires last Monday after the bolt was fired by a colleague into his chest.
He had been working at a wooden pallet factory in the Cañuelas area when the accident occurred.

Following the operation Dr Nahín said: ‘The patient has recovered well and he is now off the ventilator.’
According to Medina only three people in the world had ever survived such an injury – in the U.S., Australia and Poland.

A workplace accident could have been the final nail in the coffin for an Argentine carpenter, but thanks to a rush surgery the young handyman isn’t in a pine box.
A 19-year old carpenter, who was working at a wooden pallet factory in the Buenos Aires suburb of Cañuelas, was rushed to a hospital in the capital after a coworker accidently shot a bolt from an electric nail gun three inches into his chest.
An X-ray of the carpenter’s chest shows the eight-inch bolt piercing his heart chamber.
What was almost certainly a deadly accident turned into a miracle after two hours of surgery.
Doctors did not immediately remove the nail as it was “acting as a stopper and preventing a hemorrhage,” said the hospital’s executive director Arnaldo Medina, according to the New York Daily News .
“In the operating room, we proceeded to open the breast bone very carefully because there was a risk of hitting the nail and enlarging the heart wound. With the nail visible, we performed a manual removal of the foreign body, which was embedded in the anterior right ventricular,” said the surgeon Dr. Marcelo Nahín .
Argentine doctors said that the carpenter is now recovering from the surgery and is off of a ventilator. Photos on the Argentine government Health Ministry’s website show the man, alongside Nahín, giving thumbs up.
There is tape covering the large incision on his chest.
“Only three people have survived this injury in the world, in the United States, Australia and Poland,” Medina said. “This is the first time this type of surgery has been performed in Argentina.”
Nail gun accidents are responsible for approximately 37,000 annual emergency room visits by construction workers in the United States. Most of these accidents, however, involve injuries to the fingers and hands, Nahín said.

An x-ray revealed the nail had pierced straight through his heart muscle to become lodged in one of its chambers.
Only three people in the world have survived this type of injury, according to the hospital”s CEO, Arnaldo Medina.
And this is the first time it has happened in Argentina, he added.
Surgeons could not simply pull out the nail because it was “acting as a stopper and preventing a haemorrhage”, said Mr Medina.
Instead the man”s breast bone was carefully opened up to allow the surgeons to see and gently remove the nail.
The operation took more than two hours but the teenager has now “recovered well”, said lead surgeon Dr Marcelo Nahin.
He added that tens of thousands of US manual workers are seriously injured by nail guns each year.

Un joven de 19 años, a quien se le había incrustado un clavo en el corazón -lesión a la que sobrevivieron sólo tres personas en el mundo- fue intervenido con éxito por un equipo médico del hospital El Cruce Néstor Kirchner, de la localidad bonaerense de Florencio Varela.
“La evolución del paciente es muy buena y ayer se le retiró el respirador”, indicó el médico Marcelo Nahín, quien dirigió la cirugía practicada de urgencia el pasado lunes.
Por su parte, el director ejecutivo del hospital, Arnaldo Medina, señaló que “sólo tres personas sobrevivieron a esta lesión en el mundo, en casos ocurridos en Estados Unidos, Australia y Polonia”.
Recibió el disparo de una pistola neumática que manipulaba un compañero, mientras realizaba su labor habitual en una fábrica de pallets de madera.
Medina precisó que “es la primera vez que sucede en la Argentina, con un elemento cortante como es un clavo” y añadió que “fue muy importante la experiencia previa del equipo de cirugía, que permitió que no se le retirara el objeto inmediatamente, ya que funcionaba como una especie de tapón evitando la hemorragia”.
El director sostuvo que “contar con gente que puede trabajar en todo momento, que viene realizando cirugías cardiovasculares y trasplantes da un training para abordar este tipo de operaciones”.
Los profesionales explicaron que el clavo, de ocho centímetros de largo, atravesó el esternón y el ventrículo derecho del corazón, y quedó alojado en la cavidad cardíaca.
Cómo fue. El lunes, este joven de 19 años cuya identidad fue reservada, recibió el disparo de una pistola neumática que manipulaba un compañero, mientras realizaba su labor habitual en una fábrica de pallets de madera de localidad de Cañuelas.
De inmediato fue llevado al Hospital “Dr. Angel Marzetti” de esa ciudad, donde le realizaron una placa de tórax y observaron que el clavo estaba alojado en el corazón.
Con ese diagnóstico fue trasladado al Hospital El Cruce, donde llegó cerca de la medianoche e inmediatamente le practicaron una tomografía computada que confirmó la herida penetrante cardíaca.
El diagnóstico obligó a los médicos a realizar de urgencia la extracción del clavo y la cirugía de reparación cardíaca.
“En el quirófano se procedió a la apertura del hueso del esternón con mucho cuidado porque se corría el riesgo de chocar con el clavo y agrandar la herida cardíaca”, relató Nahín.
Y añadió que “ya con el elemento perforante a la vista realizamos la extracción manual del objeto que se había incrustado en la cara anterior del ventrículo derecho”.
En la operación, que duró más de dos horas, además de Nahín participaron el médico Jorge Troncoso, el anestesiólogo Daniel Hermoso, y los instrumentadores Marcela Caruso y Daniel Moyano.
Nahín explicó que “las pistolas de clavos son herramientas poderosas y fáciles de operar que aumentan la productividad en las tareas de clavado, pero que también son responsables de alrededor de 37.000 visitas anuales a salas de emergencias de trabajadores de la construcción en los Estados Unidos”.
Las pistolas neumáticas de este tipo causan decenas de miles de lesiones graves todos los años en Estados Unidos y llegan a provocar la muerte de trabajadores de la construcción por traumatismos graves.
El cirujano señaló que “más de la mitad de las lesiones por pistolas de clavos reportadas en EEUU ocurrieron en manos y dedos. Luego le siguen lesiones en las piernas, rodillas, muslos y pies, mientras que son menos comunes las del antebrazo o la cintura, la cabeza o la nuca, y el tronco”.
Según detalló el médico, “en ese país también se han reportado lesiones por pistolas de clavos en la columna vertebral, la cabeza, el cuello, los ojos, órganos internos y huesos, causando parálisis, ceguera, daño cerebral, fracturas de huesos y muertes”.

Devetnaestogodišnji stolar iz Argentine hitno je prebačen u bolnicu u Buenos Airesu nakon što je njegov kolega slučajno ispucao čavao iz pištolja u njegovo srce. U trenutku nesreće, mladić je radio u tvornici drvenih paleta, piše The Telegraph.
Rendgenska slika njegovih prsa je pokazala da je čavao završio u srčanoj komori, nakon čega je bio podvrgnut dvosatnoj operaciji ‘El Cruce Nestor Kirchner’ bolnici. ‘Liječnici nisu odmah uklonili čavao, kako bi spriječili krvarenje. Strano tijelo su odstranili tijekom operacije’, rekao je direktor bolnice Arnaldo Medina.
‘U operacijskoj sali smo pažljivo otvorili prsnu kost kako ne bismo dodirnuli čavao i proširili ranu. Kada je čavao postao vidljiv, ručno smo ga uklonili i pacijent je sada dobro’, rekao je dr. Marcelo Nahin te dodao da je samo troje ljudi preživjelo ovakvu povredu i to jedna osoba u Australiji, jedna u SAD-u te jedna u Poljskoj.

BUENOS AIRES.- Un equipo médico logró extraer con éxito un clavo del corazón de un joven de 19 años en el hospital Florencio Varela, quien sufrió un accidente mientras trabajaba en una maderera. El clavo se incrustó en el corazón del joven cuando se disparó una pistola neumática que manipulaba otro operario. Ambos trabajaban en una fábrica de pallets de madera en la localidad bonaerense de Cañuelas, cuando ocurrió el hecho, informó hoy el Ministerio de Salud argentino. “La evolución del paciente es muy buena y ayer se le retiró el respirador”, informó el médico que dirigió el lunes pasado la cirugía, Marcelo Nahín. Apertura del esternón El director ejecutivo del hospital El Cruce Néstor Kirchner, Arnaldo Medina, indicó que “sólo tres personas sobrevivieron a esta lesión en el mundo, en casos ocurridos en Estados Unidos, Australia y Polonia”. “Es la primera vez que sucede en la Argentina, con un elemento cortante como es un clavo”, subrayó. “En el quirófano se procedió a la apertura del hueso del esternón con mucho cuidado porque se corría el riesgo de chocar con el clavo y agrandar la herida cardíaca”, relató Nahín. “Ya con el elemento perforante a la vista realizamos la extracción manual del cuerpo extraño, que se había incrustado en la cara anterior del ventrículo derecho”, señalo el directivo al describir la compleja y riesgosa operación para salvar la vida del operario.

“La evolución del paciente es muy buena y se le quitó el respirador”, informó el cirujano Marcelo Nahin, quien dirigió la cirugía practicada de urgencia el lunes.
El operario de 19 años recibió un disparo de una pistola neumática cuando estaba trabajando en la maderera donde está empleado.
Esto provocó que un clavo de ocho centímetros de largo le atravesó el esternón y el ventrículo derecho del corazón, quedando alojado en la cavidad cardíaca, precisó el Ministerio de Salud.
“Lo bueno fue que nadie intentó sacarle el clavo antes de llegar al hospital”, agregó este jueves Nahin en declaraciones a la prensa.
La intervención quirúrgica duró más de dos horas, indicó.
El cirujano advirtió que “las pistolas de clavos son herramientas poderosas y fáciles de operar que aumentan la productividad en las tareas de clavado, pero que también son responsables de alrededor de 37.000 visitas anuales a salas de emergencias de trabajadores de la construcción en Estados Unidos”.
“Es la primera vez que se realiza una cirugía así en Argentina, con un elemento cortante como un clavo”, dijo Arnaldo Medina, director del hospital público El Cruce, de la localidad de Florencio Varela (periferia sur), donde se realizó la exitosa operación.
Según su director, el hospital cuenta con “tecnología de última generación” y se pueden “hacer desde tomografías hasta estudios de hemodinamia de la más alta calidad”.
